Most people take forms for granted--the assumption being that since forms are only drawings and we learn drawing in kindergarten, then form design must be simple. Yet this assumed simplicity is overshadowed by a vast array of human error and form filler frustration.
For example, our experience has been that between 80% and 100% of most public-use forms have errors. Yet there is no need for this to be. Forms will never be totally free of errors, but they can be made to work with a high degree of success.
